Thank you for joining the Q4D podcast. Whether we’re recording in the studio, online, or live from an event, this page provides everything you need to participate quickly and easily.

Below you’ll find two short forms, one for your basic information and one for the release required to share your episode. Each form takes only a few minutes to complete and works seamlessly on mobile or desktop.

A Few Easy Steps

Step 1 — Guest Info Form

Help us prepare for your episode

Please fill out this quick form above so we can properly introduce you, understand your preferred recording setup, and tailor the conversation to your interests.

You’ll be asked for:

Your name, title, and contact info
A short bio
A headshot or portrait
Your preferred recording method (studio, remote, on-site)
One or two Spotlight Topics; the personal area of focus for your episode

Every Q4D episode includes five core questions shared by all guests, plus a short Spotlight Topic unique to you.

This helps highlight your individuality and creative perspective.

Step 2 — Release Form

Required for recording and publishing

Before we record, we ask all guests to complete a brief release form.
This grants permission for Q4D to record, edit, and share your interview as part of the podcast series.

The form covers:

Recording rights
Use of your voice, likeness, and images.
Remote and in-person sessions.
Episode promotion and graphics.
Ownership and distribution.

You will automatically receive a copy of your signed release via email.

What Happens Next?

Once we receive your forms:

We’ll reach out to schedule your recording. You’ll receive available time slots for studio, remote, or on-site recording depending on your preferences.
You’ll receive a brief confirmation email with tech details, location info, or remote instructions.
After we record your episode You’ll be notified once your episode is edited and ready for review.
We’ll also send shareable graphics and audio clips for promotion.
